Payments and Insurance
 
For your convenience, we accept Discover, VISA, American Express, and MasterCard. 
 
If you are using insurance benefits to cover services, we will file claims with your insurance carrier on your behalf.  It will be 
necessary for you to supply us with complete and accurate information. If your insurance has changed, please update our records by 
calling us or by letting us know at your next visit. 
 
Charges for medical treatment not covered by insurance benefits are the responsibility of the patient.   
Payments are due in full 60 days from the date of service as this should allow ample time for coverage review by your 
insurance carrier. 
 
We do accept assignment from Blue Cross Blue Shield, Aetna, and UnitedHealthcare.
 
We no longer accept Cigna, Scott & White Insurance, Seton Healthplans, Integrated Behavioral Health, Tricare, Medicaid, or Medicare without secondary insurance.
